Appointments Calendar

In-Person or Zoom Teleconference

You can now schedule an appointment for either an in-person or zoom telemedicine meeting.  Once scheduled, and confirmed, we will reach out to you with zoom meeting details and information.

For Zoom telemedicine appointments, you must have a computer, laptop, tablet or phone with a speaker/mic and built-in camera, web camera or attached camera, in order for the Zoom meeting to function.

If you need to cancel or reschedule your appointment, please do so at least 2-Hours prior to the scheduled time. Please view our Terms of Service for further details.

  1. Choose your requested date and time slot.
  2. Select Appointment type
  3. If logged in, proceed with “Request Appointment”
  4. If not logged in, then Register or Login.
  5. Your appointment will be listed as pending until we confirm the details with you.
  6. If you are insured, please remember to bring your ID and Insurance Card. If you are not insured, then please bring your ID (Driver’s License, State ID or Passport) along with your SSN. You can also upload a copy of these documents here: Document Uploads Page
  7. You can visit your Appointment Profile Page (registered/logged-in Users only) to manage and review your appointments.
